Create index topics from text tags

When you import topics from another document, page references aren't imported. The script adds these. It is indiscriminate in that it mindlessly creates a page reference for every occurrence of each topic. It also ignores topic chains: of the topics finance, bank, airplane, bank, and river, bank only the bank part can be considered so the script overgenerates.
